Valid PA Drivers License or PA State ID Card

Utility Bill, Voter Registration Card, Vehicle Registration/Title Paycheck Stub with name and address, Bank Account Statement

Data is encrypted and traveling over a secured network that is regulated by the State and Federal government standards for this type of data communication.

ACT 114 Clearance Certification

The Applicant FBI background clearance check should be completed within eight weeks of applicant fingerprint submission. PDE will certify the official record by returning the Federal Criminal History Record with an imbedded commonwealth seal to the applicant via mail. Official record valid for one year.
